All BL Healthcare devices communicate directly with the secure, IP-based Healthcare Access Server. The Server runs a proprietary BL Healthcare application that creates a web-based interface for providers to manage patient interaction and care. Providers use a secure login from any internet-connected device with a browser to access the software and perform all necessary patient and patient-related management functions, including video calling. BL Healthcare has also developed applications to allow users to access Server functions such as video conferencing from iOS or Android devices such as tablets and smart phones.
The Healthcare Access Server gives clinicians access to a highly configurable tool set for designing and managing patient care. Using these tools they can create care plans tailored to individual patients or groups of patients, manage messaging and notifications based on vital sign measurement or question responses, view trended charts of vital sign data, create clinical pathways and assign multimedia educational content to be viewed directly to the patient's Healthcare Access Device.
The Healthcare Access Server software uses standard SSL and password protections ensuring compliance with Health Insurance Portability and Accountability Act of 1996.
The Dashboard provides an overall view of each clinician's assigned patients, their most recent vital sign readings and any flagged notifications. The dashboard also provides quick links to a variety of tools for patient management including messaging, video calling, notes and accessing the patient's trended charts.
Vital sign readings are stored in the system and can be displayed in a graphical format for a clinician's review. Red and Green icons highlight readings that are outside or inside certain parameters assigned to individual patients.
The Flow Chart Tool allows clinicians to create and configure a visual clinical pathway. Standard branching logic is used to establish a series of questions and response options. The flow of the questions can be contingent upon the responses of the patient allowing a clinician to better assess potential intervention needs.
Care is managed through periodic assessment sessions that provide vital sign measurement collection; assessment questions allowing clinicians to keep track of a patient's health status; periodic reminders; calls to action; notifications to clinicians if a patient is having symptoms; and educational content allowing patients to learn behaviors to self-manage their condition.
These templates were created using the Healthcare Access Server which has a tool set allowing care organizations to more efficiently and effectively handle proactive patient interaction and preventive care. The implementation of Care Pathways for targeted patient risk groups decreases the rate of ER visits and hospitalizations in post-discharge patients and patients with high-risk chronic disease.
The BL Healthcare Re-Admission Prevention Program combines the Care Pathways with a schedule of Education and Video Conferencing based on patient risk. The program offers patients and providers the tools they need to avoid unnecessary readmissions over a 30-60-90 day period. Healthcare reform is incorporating pay for performance methodologies. One of the first high-profile efforts will be focused on reducing unnecessary re-admissions to the hospital.
All Care Pathway templates are based on Standard Practice Guidelines for the management of patients with multi or specific disease states. Questions are written at or below a 6th grade reading level and avoid medical jargon and terminology. All content of the pathways are fully customizable to meet the needs of the clinician's specific patient population.
At selected intervals, the patient can be presented with educational information related to their specific condition or to general health. Educational content can be voluntary or required by the patient at certain times including at initial assessment, part of a daily session or at any interval deemed necessary for their care plan.
Original educational content created by BL Healthcare is available with each of our Care Pathways. These videos were created for specific chronic conditions, as well as for general wellness and healthy lifestyle education. These video clips can be supplemented or replaced with original content created by the health care provider using the Healthcare Access Server.
